Wrinkles develop as we age due to the breakdown of supporting structures such as collagen and elastin fibers. Sun exposure, smoking, and even facial expressions such as smiling, laughing, and frowning, contribute to the formation of wrinkles. While there are effective treatments such as dermabrasion, chemical peels, and CO2 and erbium laser resurfacing, all of these methods take layers of skin off and require bandages with significant time to recover. Non-ablative laser skin rejuvenation is a revolutionary new technology that is gentle on your skin and tough on your wrinkles.

Often there is virtually no downtime with this treatment. This not only treats your wrinkles, but also other facial conditions such as acne scarring, rosacea, facial redness, large pores and oily skin. Telangiectasias also known as spider veins or angioectasias, are small dilated blood vessels near the surface of the skin or mucous membranes, measuring between 0.5 and 1 millimeter in diameter.

These dilated blood vessels can develop anywhere on the body but are commonly seen on the face around the nose, cheeks, and chin. Dilated blood vessels can also develop on the legs, although when they occur on the legs, they often have underlying venous reflux or "hidden varicose veins" (see "Venous reflux" below). When found on the legs, they are found specifically on the upper thigh, below the knee joint, and around the ankles.

Many patients who suffer with spider veins seek the assistance of physicians who specialize in vein care or peripheral vascular disease. These physicians are called vascular surgeons or phlebologists. More recently, interventional radiologists have started treating venous problems. Some telangiectasia are due to developmental abnormalities that can closely mimic the behavior of benign vascular neoplasms. They may be composed of abnormal aggregations of arterioles, capillaries, or venules. Because telangiectasias are vascular lesions, they blanch when tested with diascopy.

A few scattered freckles on the face might be considered cute, but when years of sun damage begin to add up, the results are anything but. Most pigmented lesions can be directly attributed to overexposure of the sun’s damaging ultraviolet rays. Preventative measures should always be taken by using sun block 30+ and avoiding the midmorning and midday sun. This won’t correct any damage already done, but it can help keep pigment problems from getting any worse.

Melanin is what gives our skin its color. Dark or light complexioned, we all have varying amounts of melanin. Pigmented lesions are dark in color simply because melanin is abnormally concentrated in one area of the skin. High concentrations of melanin can be due to various factors. Some types are present at birth, but most occur with age or as a result of overexposure to the sun’s damaging rays.

Pigmented lesions can be successfully lightened or removed with modern laser technology. A laser removes pigmented lesions with the energy of light. A laser is designed to produce one or more specific light wavelength, which are absorbed by the pigment. Pigmented lesions are either lightened or removed when the laser light passes through the skin and is absorbed by abnormal concentrations of melanin. The rapid absorption of light energy causes the melanin to destruct to a point where the melanin concentration is reduced, leaving the treated skin looking uniform in color and texture. It is a safe, quick, and effective treatment.

A laser produces a beam of highly concentrated light that is directed over the area being treated. The pigment melanin located in hair follicles absorbs the light emitted by the Laser. The laser produces precisely controlled pulses of energy that are concentrated in the follicles under the skin. The energy pulse plus the absorption of light causes vaporization of the pigment and the disabling of the follicles in order to eliminate or significantly impede the hair’s re-growth.

Lasers have different wavelengths, but all work when melanin is preferentially heated to significant temperatures to destroy certain structures in the hair follicle without damaging surrounding tissues. These structures beneath the skin surface need to be significantly damaged in order to prevent new hair formation. An experienced physician can adjust settings on the lasers to achieve maximal hair reduction in the least amount of treatments. Virtually all areas of the body can be treated if the hair has at least some pigment in it. Even blonde-haired people and most redheads have some hair pigment. However, very light hair may require pre-treatment with a pigmenting agent named Meladyne for about 4-6 weeks.

Laser hair removal requires a series of treatments that are based on several factors. First, hair cells grow in cycles and can only be treated in their growth phase. Depending on the body area, an average of 30% of the hair cells may be in the growth phase at any time. Hairs are either coarse or fine, and follicles may be found at different depths and densities. Because of all these characteristics, Dr.Nayak can use different lasers and settings to achieve your goal and to individualize treatment. Most patients will need anywhere from 4-8 treatments, but some might need up to sixteen. Laser hair removal is currently the number one laser treatment performed in the United States; therefore, patients can trust that the laser hair removal procedure is a safe, fast and effective way to permanently reduce all unwanted hair.

The Ultrawave 3 at Skin Appeal is a revolutionary three-in-one high energy laser system. It has the Alexandrite laser which operates at a wavelength of 755 nanometers (nm) and is the gold standard for clients with dark hair and light skin colors. The redness and flushing which is probably the most prominent and common feature of rosacea is best treated with lasers. Linear blood vessels, those little spider veins on the face, can be very effectively treated with a long pulse duration, pulsed-dye laser or 532nm KTP Nd-YAG laser. Generalized, diffused redness can also be removed with laser treatment. Several laser sessions may be needed to complete treatment; however most patients see a significant improvement following one session. Studies also have demonstrated that laser treatments can improve the pimples associated with rosacea. If a patient makes an effort to avoid sun exposure which will aggravate rosacea (by wearing a strong sunscreen daily), blood vessels may not return for years.

Does it Hurt?

Slightly uncomfortable, not painful. Most people find the treatment tolerably annoying or slightly uncomfortable. It’s not uncomfortable enough that we ever use topical numbing creams prior to treatment. Pain thresholds vary, but rarely does anyone complain about the pain of the laser treatment.

What are the recovery times?

There is no downtime for the laser treatment. Side-effects can include swelling which lasts from 15 minutes to 4 days, but usually lasts from 2-6 hours, and some redness which usually lasts for an hour or two after treatment. Purpura, or bruising, can occur and that can last from 4-7 days. Treating the diffused generalized redness of rosacea requires some skill as opposed to treating the linear facial veins. Sometimes, it can give the illusion to people that they have been scarred, but they’re not actually scarred. The reason for this is that people with very light skin get most of the color in their skin from this redness. If a doctor over-treats that redness it can leave white marks that look like scarring, but in fact are the skin’s normal color. The way to fix this, believe it or not, is to get additional laser treatments.

How often have you gone to the beach or worn shorts in summer, all the while conscious of “those ugly spider veins”? Many a time you try to cover them up and hope that people do not look at them. It is unfortunately, a futile attempt and a losing battle!

Though seen in people of all ethnicities, spider veins are most commonly seen in fair-skinned women and usually appear in the fourth or fifth decade. They appear due to weakening of the elastic fibers in the walls of the blood vessel. On the face, spider veins can range in size from the tip of a pencil to bigger than a pencil eraser. Very often they embarrass you due to the appearance of a “permanent blush” often mistaken for an “alcoholic flush”. They may be even larger on sites other than the face. Sun damage worsens them.

The causes of spider leg veins are unclear. However, certain factors contribute to their development including heredity, pregnancy, hormonal factors, weight gain, occupations or activities that require prolonged sitting or standing, and trauma.

The most effective treatment option for visible veins is the use of a vascular laser such as the Ultrawave 3 Nd-Yag laser, which is designed to target a component of blood called hemoglobin but leaves the surrounding tissue unharmed.

These lesions have an excellent response to the new Ultrawave 3 laser. Ultrawave 3 uses a large laser spot size and significantly lower energy levels, thus it has the dual benefit of making treatment faster while greatly reducing the post-treatment darkening of the tissue. The Treatment End Point is when the vessel disappears after laser energy is applied. Several seconds later, the area becomes red, and the redness fades after a few days. Sometimes, there may be temporary purpura or purple pigmentation of the skin, similar to bruising. This purpura usually resolves in a week or two. The expected outcome is full resolution. May need several treatments to completely eliminate vessels. Fine matted telangiectasia are treated like Rosacea/Port Wine Stains. Most importantly, the results are superior to other types of laser or conventional treatments.

Laser treatments are typically done at 4-8 week intervals. Several treatments may be required.

The deep crimson color is distinctively and cosmetically very disturbing to those who carry the lesion. The lesions contain many dilated blood vessels. As such, the disease is most benign but it causes very big psychological trauma to the patient and a sense of inferiority. Once the lesions are well developing, they tend to bleed with even the slightest trauma.

Spider veins are actually dilated blood vessels. They are usually found on the forehead, nose, cheeks, eyelids, neck, and upper chest. On the legs, while most commonly found on the thigh or lower leg, they can form virtually anywhere on the leg, from the uppermost region of the thigh to the ankle.

Vascular lasers can coagulate the vessels while leaving the superficial skin healthy, and hence the clearance of the lesion is possible when treated with them. Usually Pulse dye laser, KTP laser, Nd-YAG and Intense Pulse light can treat this condition. Around 40% of patients show complete clearance of the lesions, whereas about 50% of the patients achieve clearance of around 90%. The remaining 10% of patients achieve a clearance of around 60%.

No sun exposure 7 days prior to treatment is the only precaution that should be taken.

Acne is a very common and bothersome problem that most people experience. Age, sex, atmosphere, and self-care are the most common risk factors associated with acne. Usually it is seen in teenagers, rarely later. It is seen more in hot and dusty climates. Some types of acne are caused by occupational hazards and usually seen in people working in very hot environments. Certain medicines also exacerbate acne. It is observed that consumption of spices, spirits and oil predisposes some people to acne. Acne accounts for more than 2 million visits to office-based physicians each year for 15-to-19 year-olds, but the average age at presentation for treatment is 24 years old. Ten percent of visits are for those 35-44 years old. Although there are many types of acne, acne vulgaris is the most common form, affecting an estimated 85 percent of the population at some time in their life.

Sebaceous glands lie just below the surface of the skin and excrete sebum, an oily substance that keeps skin healthy. When the tiny pathway for the sebum is clogged and bacteria called p. acnes grow in the gland, a pimple is formed. These bacteria form Whiteheads and blackheads. Comedones both open (blackhead) and closed type (whitehead) are the main lesions of acne. They are mainly seen at sites around the nose and over the forehead but can be seen anywhere over the face. Severity of the disease varies from comedones to red or yellow papules or complicated acne presentations like cysts and nodular lesions of acne. Usually initial comedones are secondarily infected with the organisms p.acnes leading to infective acne leading to scarring and pigmentation.

Antibiotics can become resistant to p. acnes. Lasers, by a photochemical reaction, can kill the p. acnes in your skin – without the use of antibiotics – and even shrink your sebaceous glands. They also reduce erythema. Lasers also stimulate collagen to improve acne scarring. Complications like pigmentation and keloid formations can also be revised. To enhance results, lasers may be used in combination with a topical photo sensitizer, Levulan®. This process is known as photodynamic therapy. The treatment is very quick and simple and is extremely well tolerated. Side effects include mild redness, swelling, and increased photosensitivity for the next few days.

Depending upon the type of acne and the number of lesions and the complications, the number of treatments varies from 1-10 treatments.

Laser treatment is designed to gently target and reduce the redness associated with inflammatory acne. It will not aggravate an existing condition. However, if the underlying causes of your acne condition change, your acne could worsen independent of the laser treatment.

Birthmarks are developmental defects in the formation of the structure of the skin. There are several types of Pigmented birthmarks. Some of them are melanocytic and others are melanotic naevi. Lasers can treat all types of melanotic naevi. The correct diagnosis of the nevus along with the correct depth of the lesion is the basic rule of treatment of birthmarks. The naevi cells and the gradual warming of the naevi cell by the laser, breaks the naevi cells in to small pieces, and these metabolites are removed from the body by the lymphatic drainage. The lesion will turn whitish or grayish but this soon disappears within the first 24 hours followed by mild redness and blanching of the lesion. Usually 3-4 treatments are needed for complete removal. There is no General or Local anesthesia required for the treatment.

Hemangiomas are vascular malformations, which appear like raised, red birthmarks. The majority occur in infants and young children with most lesions disappearing completely by age 7. While most of these lesions are superficial, hemangiomas can spread rapidly, causing considerable mental anguish to patients and their families.

On occasion, hemangiomas can be internal with life-threatening results if they interfere with the function of vital organs. Therefore, it is important that the nature of the lesion be determined early. Laser treatment should be considered at the earliest stages to arrest the progression of superficial hemangiomas. Vascular lasers can coagulate the vessels while leaving the superficial skin healthy and hence the clearance of the lesion is possible when treated with them. It is always recommended to treat the obstructive, cavernous and Nodular hemangiomas with plastic or cosmetic surgical excisions. The lesion will turn purplish like a blood clot and it is known as purpura, this will last for 5-7 days and will fade away with the fading of the lesion.

The skin is a rich reservoir of enzyme activity. There are skin care products that can enhance or inhibit the activity of natural enzymes in the skin, and there are skin care products containing enzymes that work with the natural enzymes in your skin, helping maintain the youthfulness of your skin.

In the continuous process of self-regeneration the skin sheds its top layer of keratinized, dead cells in a process called exfoliation. Treating the skin with a peeling agent can accelerate this process. Since skin cells of the top layer, the stratum corneum, are made of proteins, proteolytic enzymes are capable of decomposing and removing them. The ideal enzyme will be specific to keratinized, dead skin cells and will not attach live, healthy cells. Fruit enzymes can be used with great success to exfoliate the skin. Alpha hydroxy acids (AHAs) such as glycolic acid and enzymes perform different functions to achieve exfoliation. They complement each other when used separately. AHAs loosen and lift off the top layers of dead cells, while enzymes simply dissolve the dead skin cells off, revealing fresher skin.

Skin peels and other methods of exfoliation have stood the test of time as effective anti-aging treatments. However, very few products are formulated with pure fruit acids and natural ingredients that can produce optimal results without irritation.
